Dear Ms***, I wanted to add to my response to Ms*** complaint. The claim to her *** insurance did not include the Oral B Elec Brush ($135.00) charged to her on March 15, 2017. It is considered a device or medical instrument and is not covered by *** insurance. It was included in her overall treatment estimate as part of the take home products and included in the calculation as part of her out of pocket costs. I have attached a copy of the treatment acceptance form for your review. The charge is at the top and shows as Patient Responsibility. At this time, there is no credit on her account to refund. I am having my insurance department reach out to *** in response to their letter. Thanks, Sincerely, *** *** *** ***Senior Patient Satisfaction SpecialistAspen *** Management, Inc.Phone: ***, Extension 1329Fax: (*** ***E-mail: ***

Dear Mr***:
Thank you for sharing your concerns about the treatment you received. I am writing to you on behalf of the Amsterdam, NY Aspen Dental branded practice, owned and operated by Aspen Dental Assocof Hudson Valley PLLC I certainly do apologize for your
experience at the Amsterdam, NY Aspen Dental office
I have received your complaint statement filed with the Revdex.com. I have reviewed it and your Aspen Dental account. On June 18, 2015, you visited the Amsterdam, NY Aspen Dental office with an issue concerning your full upper ComfiLyte denture. The denture was repaired that day at no cost. As you had not been seen as a patient in more than three years, you were charged for an exam and x-ray. At that time recommendations were also made to you concerning treatment of a lower tooth that was impacting your upper denture. You chose not to move forward with treatment.
Please note that the Aspen Dental Money Back Guarantee does not apply in this case, as your denture is more than three years old. However; the seven year warranty on that denture does apply, hence the no charge repair. At present your Aspen Dental account shows a balance due of $100.00. As a courtesy, I have removed that balance. Your Aspen Dental account now reflects a zero balance due

Ron H*** Senior Patient Satisfaction Specialist Aspen Dental Management, IncAspen Dental Case # MrH***, I was quite amazed by your letter I received on August 15, You make statements in your letter that you have no proof toPlease send me a copy of my signature on agreement for $Not my finance paper'sYou don't have oneI was there you were NOTI gave your office manager a flyer that advertised a cleaning for $with 20% off for the special that you were running NO WHERE on that flyer does it offer a $charge for a cleaningThere were also specials on that flyer for dentures that were never referenced by herThere was NEVER any amountof $discussed or agreed to by meWhy would I go in there with a flyer for $for a cleaning and then agree to pay $for the same cleaning Sounds redicioulas right, because I never did I would NEVER make dental plan's with a office manager she's not a dentist and your dentist told me my teeth were fine and could use a common cleaning to match my new dentures when they were done, but the dentures were never completed correctlyI had just seen a dentist a few months before and I needed nothing done to my six (6) teethTherefore there would be no reason for a cleaning correctYou have to be a doctor to understand the medical codes your using on the billing you sent me to justify the fee and I had no idea what the codes meant until I looked them up online and found out what you are charging me $I should have never had that procedure done without my doctors consult because of my medical problems and the infection that could be spread to my heart with my high blood pressureI would have never agreed to that procedureI was frauded because I never had access to or visual to the documents she was executing from her computer on the other sideThe only thing I was told by her that I was signing was for financing through *** *** and if you have something with my signature on for $that's not my financing paper's I would like to see it I was never given anything to read until after I signed for the financing and then I was given the agreement sign by electronic key pad from the opposite side of the counterI was made to believed I was only signing for the financing that's itIf you charge $for a cleaning for teeth why are you charging $for the six(6) teeth I have? It would have been $correctI would also like to know why she sent me out to get a appointment for September when she had just completed a $cleaning on my six(6) teeth and why I have a September appointment card for a cleaning when she had already cleaned them with no tools? I've had cleaning's done my whole life and never were they like that! This is a FRAUD all she had was a pick and looked around my mouthThat”s not a cleaningIt's because this is a lie and your trying to fraud me for $that's whyI have documents to show from the Revdex.com that your company was fined by the Attorney General's for the same things.I am filing with them as wellI will see you in court as soon as I'm done filing the proper complaints with the proper state agency's *** *** - ###-###-#### *** ** ** West Sand Lake N.Y

I have reviewed the response made by the business in reference to complaint ID ***, and find that this resolution is satisfactory to me since I have paid the invoice because they are continuing to make errorsAccording to my insurance company, the procedure was not covered, that Aspen Dental told me they has it pre-approvedI received an email today from Aspen saying they are waiting to hear back from Nationwide regarding this, but since my insurance company is *** ***, I'm not hopeful.

I had three appointments with Aspen Dental; July 22nd, 26th, and Aug.3rd
On Aug3rd I was given Donald S***, a dentist not in the network
The agreement I signed shows DrAndrei P*** for all services
I should have been advised that S*** was out of network, I would have definitely rescheduled since this was not an emergencyIt was for tooth#8, resin-based surface anterior
(There is so much more I would like to mention about this apptand how uncomfortable he made me feel, just not enough time for now.)
My other major complaint is with Byron, the person who processed the agreement When I inquired why I was getting billed, he said this was not his problem It got worse, he was so rude I made this call at 10:36am on Sep8th
