We apologize for the confusion with your scantron form and risk factor documentation. It appears that the scantron did not scan properly, most likely because of stray marks on the form, and it was not properly verified. We can understand that receiving a letter from us indicating that you had risk factors that were false would be very upsetting for you. We have removed you from our mail list and you should not receive any mailings in the future. 
 
Again, we apologize, and thank you for letting us know of the problem.

customer cancels within 48 hours of their appointment, they are provided with a gift card that never expires and is transferrable so they can reschedule at their convenience.   Because of the inability to get her problem explained or resolved via customer service, we will process this refund and apologize again for the negative experience.

Revdex.com Complaint ID: [redacted] Dear Mr. S[redacted] We are in receipt of the complaint that was filed with the Revdex.com, as well as your subsequent letters of complaint that were sent to our corporate office. We regret that you had an unfavorable experience with our company and appreciate the opportunity to clear up any miscommunication. According to your letter and our records, our mobile unit provided correct information regarding the recommended Medicare benefit of an Annual Wellness Visit. An Annual Wellness Visit is primarily a consultative service designed to identify risk factors and to create a personalized prevention plan for good health. By definition an Annual Wellness Visit is only allowed once per year. it is not a routine physical. In fact, Medicare does riot cover a routine physical. This can be confirmed by calling Medicare at (800) MEDICARE. A Medicare provider is required to inform a patient in advance of their financial responsibility prior to the service. They are also required to obtain informed consent in the form of a Patient Agreement and Consent for Treatment. We have met both of those requirements. As you reported our team indicated that the Annual Wellness Visit would be covered by Medicare and it was covered at 100% with no out of pocket expense to you. We also obtained your signature on a Patient Agreement and Consent for Treatment form that gave permission to complete the Annual Wellness Visit. In addition, you provided a copy of your Medicare card for the purpose of our billing the service to your insurance carrier. We have attached copies of these documents for your review. There are also specific guidelines that Medicare dictates for the Annual Wellness Visit itself. Our Annual Wellness Visit has been reviewed by Medicare and found to meet all Medicare guidelines. We have attached a document that Medicare has published which explains the Annual Wellness Visit requirements. We are also providing a copy of your medical records from your Annual Wellness Visit so that you can see that the requirements were met.  Life Line contends that we met our responsibility to provide proper informed consent. We have not billed you for the service because Medicare paid for it in full. As far as the bill that was received from your personal physician, that is a matter that would need to be taken up with him. All Medicare providers have the same responsibility to inform a patient in advance if they are going to incur charges. We hope that the documents attached for your review will clear up any confusion regarding your services. Life Line does not intend to refund Medicare for the service that we provided. We do not intend to inform Medicare that service that billed "was not a physical" because we didn't bill for a physical. We correctly billed for an Annual Wellness Visit and as previously stated a routine physical is a non-covered service. We did indeed provide the service with your permission and payment was warranted. We also do not intend to pay your physician's bill as that is a situation that would need to be resolved with your physician. Life Line takes great pride in our contribution to preventative healthcare. We strive to be compliant with all health care laws and do not condone fraud in any way. We will be happy to answer any additional questions that you might have.
