Please find attached a copy of Mr [redacted] membership agreement with the SNAP Fitness in [redacted] , CO Please note that Mr [redacted] initialed the membership agreement under the section titled Cancellations Stating that Mr [redacted] understands that our facility requires a day letter of cancellation be delivered to the club stating he wishes to cancel his gym membership Please also note the section titled "Membership Freeze" This section of the the membership agreement clearly spells out the requirments for placing a gym membership on hold1st that a membership may be placed on "Freeze" for months out of any calendar year2nd written notice must be given to "Freeze" the account by the 25th day of the month3rd the customer will incur a $processing fee per month that the membership is "Frozen"4th a membership can not be cancelled while the account is "Frozen" On 4/28/Mr [redacted] requested to have his membership placed on "Freeze" for the months of May, June, and July Per our membership agreement Mr [redacted] was obligated to pay for the month of August After having his account "Frozen" for months Mr [redacted] cancellation notice was not received untill 8/4/ It should also be noted that Mr [redacted] had previously been a member of the SNAP Fitness in [redacted] CO from 3/5/- 4/28/ During his previous membership he provided us with day written notice when he wished to cancel his membership
