As stated previously, these records include other members' personal contact information and can not be released with out a legal request to do so. We value our customer's right to privacy. The cancellation policy is stated in two different sections of the contract that is signed digitally by all members upon joining. Other fitness & health businesses also require similar notice when cancelling with their facilities.

We process cancellations this way as it provides a receipt of cancellation for you in the event that there’s a discrepancy. We do require a 45-day notice with a pro-rate to be collected from the date the cancellation form is signed or from the date the mail is postmarked.
You are still within your one-year obligation until 8/*/16. If you decide to cancel your membership prior to 8/*/16 a $60 buyout fee associated with your Green membership will also be collected. You must pay the cancellation prorate and buy out fee at time of cancelling in order for your cancellation to be processed.

try to be as transparent as possible. We do list on the website under each membership class the terms associated with Blue, Green, or Gray memberships. Blue and Green state in details that the agreement is for 12 months. It also states this fact in the FAQ section of the website:The cancellation policy and 12 month commitment is also listed on the membership contract that all members are required to sign. 
 
What is the cancellation policy?
Our Gray membership is a no-commitment agreement that allows you to cancel at any time with 45 days notice.
Our Blue and Green memberships are 12-month agreements. You can cancel the agreement within the first year by giving 45 days notice and by paying a $60 buyout fee. After the first twelve months of membership, you can cancel with 45 days notice without a buyout fee.

We're sorry that you feel we have hidden the annual maintenance fee. That is certainly not our intention. The receipt you received after joining Blink states when your annual maintenance fee will be charged and how much it will be. We also list language regarding the annual maintenance fee in the membership agreement as well online in the FAQ section of the website. Finally, it is listed in the details below each membership class online for all members to review. We have recently posted signs in all facilities detailing when the annual maintenance fee will be charged as we changed the date from the 1st to the 3rd of a member's designated month. Transparency is our goal.

 A medical condition prevented me from using the facility. I attempted to cancel prior to November ** and was unable to speak with management regarding the circumstances regarding the circumstances  surrounding my cancellation and request for membership freeze. Management was unresponsive to my repeated phone calls and messages which started during the first week of November. It is disgraceful that this company makes no concessions for patients with medical conditions precluding their use.

 They are still ignoring my concern -- that I had intended to cancel it a while back, and was told it would be done, but was never given any thing to sign. I left for college right after this event and was still being charged, and had no opportunity to go to the gym to cancel my membership. It's bt that they keep mentioning this form I was supposed to have signed when I was never given a form to sign but was still being charged. I have not set foot in Blink Fitness since August.

Thank you for your message. When a member joins Blink they check off that they agree to all terms and conditions. This includes the cancellation policy.

We received no cancellation request from this member. As a result, the account continued to be active. Eventually the account began to accrue a balance as the credit card was declined. Several email notifications and phone calls were sent. Prior to cancelling the account for final notice collections, we reached out to the members bank to see if a new credit card was issued for the authorized account. The bank gladly provided the new credit card information because the member did not request her bank to not do so. Therefore, the balance was then paid using the new credit card. If the member requests her bank to stop payment for the credit card on file, this will not cancel out the member's Blink fitness account. The account will cancel for final notice collections after 3 months of unpaid dues accrue. Unless, the member requests cancellation of the account via one of the aforementioned ways. 
 
This account is currently set to cancel for final notice collections on 11/**/16 for 3 months of unpaid dues for the following dates: 9/*/16, 10/*/16, and 11/*/16. However, we understand this member has experienced medical issues and has no intention of using the membership moving forward. Despite Blink memberships not being based on usage, we have decided to cancel out the account effective immediately and waive the 3 month balance as a customer service gesture.

For the record, on my last day at the [redacted] Street location I was told the manager could
not close the account and therefore would not give me the form.
I was told by the manager at the [redacted] Street Blink that I could not sign the cancellation form on the spot because you all were charging my account.
But, I will file a formal cancellation form with the expectation of this account being closed with no further balance or contact from Blink Fitness.

Per the terms of the agreement, Buyer shall not be relieved of Buyer's obligations to make payments agreed to, and no deduction from any payments shall be made because of Member's failure to use the Club's facilities. Buyer and/or Member is obligated to pay any collection and/or legal costs incurred by Blink for collection of any fees. Blink reserves the right to charge balances and overdue balances to their current account under the Electronic Funds Transfer Authorization. Buyer is responsible for EFT. Per our Member Services, payment reminder phone calls and emails were sent to the member. Balance of $78.39 remains on account.

Blue memberships allow members access to all locations except for [redacted] street, and any future club opening in Manhattan that we choose. Blue members who joined prior to January [redacted], 2015 were...

allowed access to the listed Manhattan clubs for a set period of time as a courtesy but that has since ended. As external costs such as those to lease and operate the locations continues to rise along with an increased volume of members using the popular facilities, access levels were adjusted accordingly. 
The $20 Green membership at Grand Central you referenced is available for Pre-sale members only. These members are not billed monthly and are not able to use any location until the Grand Central location officially opens. The $20 rate is retained by the member for 1 year only, when they are subject to an increase to the standard $25 rate. 
We are happy to upgrade you to the Green access level if you so choose, allowing you access to all Blink facilities including [redacted], or downgrade you to Gray, allowing you access to one location only. The cost of a Green membership is $25/month and Gray at the [redacted] location would be $25/month as well.
Please inform the front desk of any facility if you would like to make a change to your membership class.

Thank you for your inquiry. This member joined our 54th street location during pre-sale as a Green access member for a $1 start-up fee. All pre-sale members are not allowed access to any location while their home facility is not open. The reason is they have not paid any...

membership dues to utilize a facility and they have been given an exclusive $20/month rate instead of the $25/month rate given to non-pre-sale members who join open facilities. Our system experienced a glitch for this particular membership which allowed him access to facilities despite being a pre-sale member. He used multiple facilities sometimes twice a day for 3 months prior to the opening date of the 54th street location with out paying any membership dues. He essentially had 3 months of free time to work out in Blink facilities. When joining all members are required to sign a membership contract. Blue and Green memberships do include a 1 year obligation. This member did sign an agreement, agreeing to all terms and conditions. Three months after joining a blink, the annual maintenance fee is charged which is $39 + tax. The language regarding the annual maintenance fee is listed under each membership class upon joining, in the FAQ page on the website, as well as in the membership terms and conditions. We feel we gave more than adequate notice of our monthly fees, the 1 year obligation, and the annual maintenance fee. 
Language on the site: 12-month agreement, billed monthly on the 1st.$39 annual maintenance fee is charged on the 3rd day of the 3rd month after joining. 
Language on the FAQ page: The annual maintenance fee is $39 and charged on the 3rd day of the 3rd month after joining and each subsequent anniversary of this date (i.e. January joins are billed April 3rd, February joins are billed May 3rd, etc.).  It is invested straight back into our locations to maintain equipment and ensure the gyms are always fresh and clean.

[redacted] joined during presale on October **, 2014 at our advertised $1.00 pre-opening startup fee and $15 per month ($16.05 including tax). $15 per month prices were advertised on all marketing collateral and webpage in order to process the sale. As per the terms of the member's signed agreement, member agreed to pay $15 beginning January [redacted] 2015.As per [redacted] signed membership agreement, Blink will honor our cancellation terms by providing 45 days notice and collecting the final prorate for these usage days. Member signed her Request for Cancellation form at Blink's front desk on 6/*/2015, agreeing to pay $9.32 final prorate and providing 45-day notice to cancel her membership. The terms and conditions regarding Blink's cancellation and billing policies were agreed to and signed by the member, and being honored by all members in all locations.

front. A credit card must be entered along with an expiration date and security code. The Security code is the 3 digit code on the back of the credit card. Memberships are not based on usage with Blink. A member pays their monthly fee and may utilize the facility as much or as little as they choose. A member does not need a key fob to use a Blink. They simply need to provide a photo ID and the front desk will be able to locate their account to allow entry.  
If this member is claiming fraud on their credit card. They must report this to their bank who will work with our disputes department to resolve the matter. We do have a signed contract on file. The account is currently cancelled due to final notice collections as well despite several attempts to contact the member.
