Regarding the recent response,  if the member is able to provide proof of medical leave, we will be able to remove the balance from collections. If not, the balance is valid, as there were attempts to contact her regarding the balance on her account prior to it being sent to collections.Thank you.

We have confirmed that the member sent a letter on 1/23/16.  Last charge should have been 2/1/16 for both accounts. A refund form is attached for the 3/1/16 charges, totaling $19.98. If the member can provide proof of cancellation prior to 1/23/16 cancellation letter, we can adjust the refund...

Hello,Our system shows that the member has already spoken to our member services department on 3/29/16. A refund form was sent directly to the member for $269. We are awaiting receipt of the form back from the member in order to issue the refund.

I have submitted a request for a refund for the annual fee of $39 as it was drafted early.  There will not be a refund for the additional fees you are requesting.

Refund check was issued 12/1 and sent out via USPS 12/2. See attached notes from accounting department.

Both of the accounts have been terminated making the last month due is for May.  There will not be a refund issued.

Corporate office has already removed the balance from this account and the account was terminated.

Hello,We show a cancellation record as of 6/22. The company will issue arefund for the 8/1 charge of $9.99. However, all charges are valid prior tothat time frame due to the 30 day notice as signed in member’s agreement.We show that the membership is currently canceled.

[redacted] joined the gym on 8/22/16 with a Premium 12 Month membership @$19.99.  This membership includes access to all the gym facilities as well as any and all classed offered at the gym. There is a 12 month commitment which can be broken by early cancellation with a $59 Buy-Out...

fee.At the end of August a decision was made to eliminate 2 of the previously scheduled yoga classes due to low attendance, however there are still yoga and body pump classes on the schedule.  The fact that the class schedule changed does not void the Membership Agreement as specific classes were not purchased, only the availability of classes and use of the gym equipment is paid for.  Since [redacted] states she only wanted the particular classes that are no longer in the same place on the schedule she observed in August she has requested cancellation and her cancellation request has been processed according to the terms of the Agreement she signed when she joined. RespectfullySheila K[redacted]Billing Manager, Fitness Evolution Home Office

Our notes show the member was attempting to do trade in promotion. The attached notes indicate the member spoke to our corporate office and the member was advised to send in proof of membership cancellation from the other club, so we honor the promotion.
