complaintant received invoice from Quest Diagnostics for This bill is Invoice* ***the bill is for lab work done on April 28, at Quest Diagnostics located in the *** *** Center located at *** ** *** *** *** *** ** *** Since then Quest has moved to other location in Park Ridge, Il.I am retired from the City of Chicago and is provided a card from *** *** *** *** stating that all lab charges are freeThe card displays a Quest Diagnostics number ***On this occasion, April 28, I provided Quest the card, which they made a copy of it, relating their is no charge for the Lab workIn the meantime I have received a bill for $for services covered by my cardThe services are free and I will not be paying thisEvery time I go to Quest, provide them with the card the same thing happensI'm sick of itPlease resolve it

I had my blood taken by my GP for thyroid levelsThe doctors office used Quest diagnostics as alwaysThey usually send through my insurance and it covers 80% of the costBut Quest did not properly contact and file with my insuranceI have called about this numerous timesThey are now having a bill collector call meThey are charging me $for my bloodwork one timeRidiculousSomeone did NOT do their job properlyI demand this issue be handled appropriatelyI will
Not be penalized and pay a ridiculous amount for one tube of blood to be rested because people cannot do their job correctly and file with my insurance which is *** *** They do take my insurance and the doctors office assured me of thatI want a drastic reduction in price and this bill collector to stop harassing meI have PERFECT credit and I will not tolerate incompetence and fraud or victimization of a patientThis is unethicalIt is NOT ok for them to threaten to contact the credit bureau and ruin

Quest Diagnostics received double payment for labwork done on performed on 5/8/ They billed me for $out of pocket from my Flex Spending Account for the services Additionally, my insurance plan, *** ***, later paid the full amount as well, including the $ I am due a refund in the full amount paid out of pocket as the services were already covered and paid by my insurance I have spoken on multiple occasions by phone and email the Katherine M***, Physician Account Manager with Quest Diagnostics who has said she will work on the issue but has never resolved it It is now October and I have still not received a refund I have also spoken with the Quest Diagnostics billing department in Baltimore which covers our region at Phone * ***  They have failed to refund my double-payment Katherine M*** phone number is *** her email is *** and her address is *** *** *

My insurance paid Quest Diagnostics $on 12/24/for services rendered on 11/5/ Quest agreed to charge me only $for the services, and was supposed to refund me the $overpayment Instead, they turned the account over to a collection agency - permanently damaging my credit and causing my credit card interest rates to increase because of the bad mark on my credit Invoice ***

I have to make customers AWARE of their horrible business practicesIf you pay over the phone, they charge you twice the same amount by "mistake" you call and it takes 4-to issue a refundThey can double charge you immediately but to give you your money back 4-6?? Do they really have that many claims to process???
They have charged me for tests that WERE NOT IN THE DOCTORS ORDERS, however doesnt matter how many time you call, who do you speak with, they will still charge it and threaten you to send it collection, even to my year old motherIs pointless to prove to them, fax them the documents, they treat with disrespect and complete disregard of your claimBottom line even if the test wasnt perform because it was not in doctors orders they still charge you for this ghost testing, and complain in the billing department with the document in hand with the doctors orders, means NOTHING TO THEMIs despicable that UPMC deals with this business, I will suggest you look elsewhere for your lab work, and mention it to your doctorThis business is VERY FISHY

On April I contacted Quest by e-mail requesting a copy of my ABN for a PSA test dated Novenber The invoice number is *** Lab Code TAM I also notified them that my doctors office submitted a new *** *** *** to them on three separate occasions but that they did not resubmit this to Medicare They made to response to their billingg problems I e-mailed them again on April and again received no response Poor customer service Ignoring information submitted to them

No matter how you pay, at the doctor, with insurance, without, with credit cardOne thing is for sure, Quest Diagnostics WILL send you a bill, regardless if you have paid for it, insurance covered it - it dose not matterThese greedy PIGS will always send you a bill for either the same amount or more, something ridiculous like $for an A1C test and claim its YOUR responsibility of the billEven though your insurance already paid for this $testGREEDY PIGSYou WILL get tired of calling their billing department and waiting for a call backThen, they will send you to collection and chase you until the end of time with HORRIBLE attorney trolls they hire

Will never go there again Blood work done two weeks ago Had an MD appttoday to go over results Lab work was never faxed over It's not easy to get an year old out for appointments and it was a waste of time Still waiting on hold on the phone to speak to someone Awful service

I received a bill from Quest Diagnostics for $ I paid the bill but appealed the claim with my insurance company The appeal determined that I did not owe the $I have requested a refund multiple times but they keep giving me the run around

Quest has continued to invoice me regarding a $co-pay that was paid months agoThey have given it to a collection agency *** *** *** *** *** Original invoice - #*** Date 10/18/
Check to Quest for $processed by First Tennessee Bank on 11/24/I have copy of First TN payment process paperwork
I received the "Fourth Notice" from Quest on 2/7/after numerous phone calls/FAX memos with proof of payment
Now, I have the collection agency harassing me over the $This needs to stop immediately
I would like a refund of the $in consideration of all my time and effort to provide Quest and *** *** with proof of payment

Added more testing without constant

I went to get blood work today, 8/28/at Quest in Houston near Willowbrook mall I had two blood work orders, one for biometric screening paid for by my employer, and one doctors order to be paid by my insurance The phlebotomist said the doctors order would be approx $and I needed to give her my credit card before she could draw my blood I told her Quest always sends me the bill, after my insurance has paid and adjusted for what I own, which would be much less than $ And I always pay my bill promptly when I get it She well that's fine, but refused to draw the blood work for my doctors order unless I gave her a credit cardI had a Quest insist on this once before a couple of years ago, and I did give them my card It was compromised within a week and was used to buy alcohol and hair products all over Houston before my bank shut it down Now I will never give it to them again, and also because I don't trust their billing department enough to allow them to pay themselves with my cardAfter I declined to give her my card, she proceeded to draw the blood for my bio-metric screening paid for by my employer She jammed the needle in my vein so hard it left a big bruise Then she put tape around my arm so tight it also left bruisesI will NEVER go to Quest again I don't know how patients put up with this treatment Quest does not care about their customers/patients and only cares about their money Phlebotomist should not be concerned with bill collection, only with blood collectionAnd they shouldn't treat patients like they are criminals who are trying to rip off services from Quest

I went to my gynecologist because I had an ovarian cyst that rupturedWe tried to get to the source of the problem, and I asked for a hormone testQuest Diagnostics ended up testing for a general health panel ($243), a lipid panel ($53), a bilirubin direct ($20), Vitamin D ($241), Vitamin B($115), Folate ($127), GGT ($29), Fructosamine ($60), FSH ($135), LH ($135), Hemoglobin ($74), Iron ($26), LD ($24), Phosphorus ($19), Prolactin ($166), T($150), and Uric Acid ($18)The vast majority of those tests have NOTHING to do with hormone levelsThe total? $With my insurance, I "only" had to pay $194, which is still no small numberI also once had a doctor send basic, routine STD screenings to Quest Diagnostics and came back with a $billThey over test and overpriceIt is insane how much this all costs, and it's a scamIf you don't pay the bill within a month, they also start calling and leaving voicemails several times a weekI don't care what illness I have, next time I will refuse tests under all circumstances because it is outrageous and abusive towards sick people — and even people who are in good health!

Quest diagnostics repeatedly attempted to collect payment for services paid for by my insurance; $over what should have been our co-payI spoke on the phone to billing multiple timesI had a way conversation with my insurance (***) and Quest, during which *** sent Quest an explanation of benefits and confirmed that they covered (read: had already paid for) certain services, and the amount owed was $less than what we were being billedWhen Quest called to collect the debt once more, I told the representative that I would be happy to pay my bill once I received a copy of the bill for the correct amountAgain and again, I received the same bill, for the same amount, in the mailInstead of correcting the issue, Quest sent the account to collections

I had a Lyme disease blood test that was conducted on July 11, in my doctor's office Later, I received a charge of $from Quest Diagnostics Labon the CPT code *** for a *** *** DNA, Real-Time PCR test, which has not been cleared or approved by FDA, and isn't covered by my insurancePlease reference the web link about CPT code *** at
*** I contacted my doctor's office and was told that they did not order the CPT code *** My doctor only ordered the typical lyme disease blood tests, which are covered by my insuranceQuest Diagnostics did not have patient's and doctor's approvals for CPT code *** before the blood testes were conductedQuest diagnostics should stop sending me the bill, which includes the charge of $for the CPT code ***

This lab has a fundamental issue with billing ALL patient insurance, which includes secondary policiesI'm frustrated in dealing with the lab directly for every billing, they claim they don't have my secondary insurance information on file and that billing a secondary is a COURTESY and not their policyThis is unacceptableI pay good money to have double coverage with medical insurance to avoid out of pocket costs from providersMy deductible and out of pocket are met on my secondary insurance so there should be NO BALANCE BILLING to me

I was told by the Quest employee that the most I would have to pay for my blood work was $I told her I didn’t want to do the test if it would come back that I owed money laterThe employee said “no, it says right here maximum patient will be responsible for is $5” and two other times said she gaurunteed I would not owe anything elseA month later I got a bil for nearly $I called to speak with their billing departmentThey reprocessed the claim with my insurance but again came back and said I owe $I spoke to a supervisor in the billing department and repeated that I would not have had the test done, but your employee GUARUNTEED that it would only cost $I told her this was a mistake by her company and they needed to eat the cost for that reasonShe said they do not do that, I needed to pay, but that she would contact the facility manager to do retraining on billing for their employeesReputable companies own up to their employees mistakes and make things right with the customerWill request to use a different provider from now on

I saw my physician for my annual visit and they ordered lab work I had the labs drawn in their office, which apparently is a Quest Lab The tech who drew my lab had all of my information They failed to tell me my insurance had recently become out of network with Quest As a result, I received a bill When I contacted my insurance company, they informed me all labs done as a part of my annual exam should be covered 100%, and I would need to ask my doctors office to code it correctly I contacted my doctor's office, who said it had been coded incorrectly and they would send a letter with the correct coding During this process, Quest Lab sent my bill to a collection agency Today I spoke to Quest, and for the first time, they notified me my insurance was out of network They did not notify me prior to drawing my blood, at my doctors office My doctor's office was unaware my insurance was now out of network The tech who drew my blood should have given me the information, an
